Registration has begun for the 2011-2012 school year. Please download the appropriate form below and submit it to the school office to reserve your child's seat today! Kindergarten - 5th Grade Registration Form *All children entering Kindergarten or for the first time are required to turn in a medical form.
*All children in Kindergarten and Grades 2 are required to have an oral health examination.This requirement includes all public, private, and parochial schools.
*All children in Kindergarten and Grades 2 in a public, private, or parochial school and any student enrolling for the first time in a public, private, or parochial school are required to have an eye examination.
